Can Male Factors Contribute to Recurrent Miscarriage?
Recurrent pregnancy loss is usually investigated as a couple. While many miscarriages are unrelated to the male partner, sperm DNA damage, selected genetic abnormalities and underlying male reproductive problems can be relevant in some couples.

The male partner should not automatically be left out of the investigation
Pregnancy loss has many possible explanations and often remains multifactorial. Current fertility guidance supports considering assessment of the male partner when a couple has experienced recurrent pregnancy losses.
Recurrent loss generally warrants a couple-based assessment
Contemporary guidelines commonly use two or more pregnancy losses as the point at which recurrent pregnancy loss evaluation should be considered. The exact investigation depends on the pregnancy history, previous genetic testing and fertility circumstances of the couple.
Pregnancy factors still matter most
Chromosomal abnormalities within the pregnancy, uterine factors, maternal age and selected medical conditions remain important causes or associations and require appropriate assessment.
The male contribution can be missed
A man may have a broadly normal conventional semen analysis while other sperm or genetic factors remain relevant to the couple's reproductive history.
Testing should answer a question
The aim is not to order every available fertility test. Investigation should identify information that could clarify the diagnosis, influence counselling or change management.
“His semen analysis was normal, so it cannot be a male factor.”
Not necessarily. Semen analysis evaluates parameters such as sperm concentration, motility, morphology and semen volume. These are important fertility measurements, but normal values do not guarantee normal sperm DNA or chromosomes.
Conventional semen parameters also do not reliably predict recurrent miscarriage on their own.
- A semen analysis can still identify a coexisting fertility problem.
- Previous abnormal results should be reviewed rather than relying on one test.
- Severe oligozoospermia or other abnormalities may alter genetic investigation.
- Normal conventional parameters do not automatically exclude further male assessment.
What male factors may be relevant?
An association does not necessarily mean that one factor caused a miscarriage. Assessment looks for reproductive findings that may be clinically relevant and potentially modifiable.
DNA fragmentation
Higher sperm DNA fragmentation has been associated with pregnancy loss in multiple studies, although the clinical meaning of an individual result requires careful interpretation.
Chromosomal rearrangements
A balanced structural chromosome rearrangement in either parent can sometimes lead to embryos with an unbalanced chromosome result.
Varicocele
A clinically significant varicocele can be associated with abnormal semen parameters and increased sperm DNA fragmentation in selected men.
Paternal factors
Paternal age, smoking, obesity or metabolic health, heat, illness and selected environmental or medication exposures can be relevant to sperm health.
What might the male partner actually need investigated?
Investigation should be targeted. Not every couple requires every test, and previous miscarriage or fertility investigations may determine which male tests add useful information.
Reproductive history
Previous pregnancies and losses, time trying to conceive, prior fertility treatment, previous children, illness, surgery, medications, testosterone or anabolic steroid exposure and lifestyle.
Male examination
Testicular size, reproductive anatomy and examination for a clinically significant varicocele or another relevant male reproductive abnormality.
Semen analysis
Existing results are reviewed and repeat testing may be useful where results are abnormal, incomplete or substantially variable.
Sperm DNA fragmentation
May be considered particularly in otherwise unexplained recurrent miscarriage or where recurrent loss occurs together with infertility.
Targeted genetic testing
Karyotype or other genetic testing may be appropriate according to the pregnancy-loss findings, semen result, reproductive history and previous genetic investigations.
Additional testing when indicated
Hormone testing, ultrasound or other investigations are generally driven by the male history, examination and semen findings rather than recurrent miscarriage alone.
Why is sperm DNA fragmentation discussed after recurrent miscarriage?
Sperm DNA fragmentation tests assess damage to sperm genetic material. Research has repeatedly found higher levels of fragmentation among some men in couples experiencing recurrent pregnancy loss.
Higher levels have been linked with miscarriage
This is an association at a population level. It does not establish that an elevated result caused a particular miscarriage.
There is no single universal DFI cut-off
Different laboratories and assays measure sperm DNA damage in different ways. Results need to be interpreted using the method and reference range of the laboratory performing the test.
An abnormal result is a reason to investigate
The useful question is whether a potentially relevant male factor such as a varicocele, smoking, illness, medication exposure or another reproductive problem can be identified.
Although several interventions can reduce sperm DNA fragmentation in selected circumstances, evidence that lowering the laboratory result itself prevents future recurrent miscarriage remains limited. Treatment should therefore be directed at a clinically meaningful diagnosis rather than the DNA-fragmentation number alone.
When might male genetic testing matter?
Genetic investigation should be coordinated with the couple's wider recurrent-miscarriage assessment. The most useful test depends partly on whether chromosome testing of miscarriage tissue has already identified an abnormality.
Parental karyotype
A karyotype examines the number and large-scale structure of a person's chromosomes. A small proportion of couples with recurrent pregnancy loss have a parent carrying a balanced structural chromosome rearrangement.
Because the carrier may be completely healthy, the finding is often only discovered during reproductive investigation.

The male karyotype, Y-chromosome microdeletion testing, CFTR testing and expanded carrier screening answer different clinical questions.
Tests commonly used for severe sperm-production abnormalities or congenital reproductive obstruction should not automatically be ordered simply because a couple has experienced miscarriage.
Genetic counselling may be valuable when a clinically significant chromosome or inherited genetic finding is identified.
The male result needs to be interpreted alongside the pregnancy history
A reproductive-urology assessment does not replace review by the couple's fertility, obstetric or recurrent-miscarriage team.
Information from the pregnancy can sometimes be more informative than performing increasingly complex tests on either partner without a clear diagnostic question.
- Were chromosome studies performed on miscarriage tissue?
- Were losses chromosomally normal or abnormal?
- Is there also difficulty becoming pregnant?
- Have IVF or ICSI cycles repeatedly failed?
- Are semen abnormalities or a male reproductive diagnosis already known?
- Does either partner have a relevant genetic or family history?
Finding a male factor does not automatically mean surgery, medication or IVF
Management depends on what is actually found. The aim is to treat a clinically relevant diagnosis where reasonable rather than treating a laboratory marker in isolation.
Potentially useful approaches
- Stopping smoking and addressing relevant recreational exposures.
- Reviewing medications and exogenous testosterone where relevant.
- Optimising general and metabolic health.
- Allowing recovery after significant febrile illness where appropriate.
- Assessing and treating a clinically significant varicocele in selected men.
- Genetic counselling when a meaningful genetic abnormality is identified.
- Coordinating the male pathway with the treating fertility team.
What an abnormal result does not automatically justify
- Unproven supplements or medications solely to lower a DFI number.
- Varicocele surgery when there is no appropriate clinical indication.
- Assuming IVF or ICSI automatically eliminates sperm-related risk.
- Routine surgical sperm retrieval simply because DNA fragmentation is elevated.
- Repeated broad genetic panels without a defined clinical question.
- Delaying the other partner's fertility assessment while investigating the male.
What if a varicocele is found?
Varicocele is one potentially modifiable male fertility factor and can be associated with increased sperm DNA fragmentation. However, finding a varicocele does not mean it caused previous miscarriages, and repair is not indicated for every man. Clinical examination, semen findings and the couple's wider fertility situation determine whether treatment is reasonable.

A useful assessment moves from the pregnancy history to targeted male investigation rather than beginning with an indiscriminate list of tests.
Review the pregnancy-loss history
Number and timing of losses, previous chromosome testing, pregnancies with this or previous partners and any fertility treatment are reviewed.
Review the male reproductive history
Fertility history, medications, testosterone exposure, illness, surgery, testicular history, lifestyle factors and previous semen testing are considered.
Examine and establish the male fertility phenotype
Examination and semen results help determine whether there is a conventional male infertility diagnosis in addition to the recurrent pregnancy loss.
Select additional testing where it can add information
Sperm DNA fragmentation, karyotype, hormones or other targeted investigations can then be considered according to the clinical context.
Identify potentially modifiable factors
A clinically significant varicocele, medication exposure, hormonal suppression, lifestyle factor or another male reproductive problem is addressed where appropriate.
Coordinate the next reproductive plan
Male treatment, natural conception, IVF/ICSI, embryo or miscarriage genetic information and reproductive timing should be considered as part of one couple-based plan.

Can sperm problems cause miscarriage?
Male reproductive factors can be associated with miscarriage in some couples, particularly certain chromosome abnormalities and increased sperm DNA fragmentation. However, miscarriage has many possible causes and an abnormal male result cannot usually prove that the sperm caused a particular pregnancy loss.
Should the male partner be tested after two miscarriages?
Current guidance supports considering male assessment in couples experiencing recurrent pregnancy loss. The appropriate tests depend on the pregnancy history, whether miscarriage tissue has undergone chromosome testing, semen findings and whether infertility is also present.
Can a normal semen analysis rule out a male cause?
No. Semen analysis measures sperm concentration, movement, morphology and other conventional parameters but does not directly assess every aspect of sperm DNA or chromosome integrity.
Should sperm DNA fragmentation be tested after recurrent miscarriage?
It may be considered particularly when recurrent miscarriage remains otherwise unexplained or occurs together with infertility. It is not required as a routine test for every man undergoing an initial fertility assessment.
What DFI level is considered abnormal?
There is no single threshold that applies to every sperm DNA fragmentation assay. Different laboratory methods have different reference ranges, so the result should be interpreted according to the test performed and the clinical circumstances rather than using a universal internet cut-off.
Can sperm DNA fragmentation be improved?
Some potentially relevant factors can be addressed, including smoking, selected exposures, recent illness, certain medications and clinically significant varicocele. Whether improving the DNA-fragmentation result itself reduces the future risk of recurrent miscarriage is less certain.
Does a varicocele cause recurrent miscarriage?
A varicocele can be associated with abnormal semen parameters and sperm DNA fragmentation, but finding a varicocele does not prove that it caused previous miscarriages. Repair is considered selectively based on the clinical varicocele, semen findings and overall fertility plan.
Will IVF or ICSI solve sperm DNA fragmentation?
IVF and ICSI can overcome several barriers to fertilisation but do not automatically remove all sperm-related biological factors. Assisted-reproduction planning should therefore be coordinated with the fertility specialist and embryology team.
Should testicular sperm be used when DNA fragmentation is high?
Testicular sperm has been investigated in selected men with elevated sperm DNA fragmentation, particularly after previous assisted- reproduction difficulties. It is not a routine treatment for recurrent miscarriage or an isolated abnormal DFI result, and the potential benefits and surgical risks require individual discussion.
Does the male partner need a karyotype?
Sometimes. The value of parental karyotyping depends on factors including previous chromosome testing of miscarriage tissue, identified unbalanced chromosome findings, fertility history and other genetic information. Targeted genetic assessment is preferable to automatically ordering every available fertility genetic test.
Should both partners be investigated at the same time?
Usually, yes. Male assessment should complement the pregnancy-loss and fertility evaluation of the other partner rather than delay it. Information from both sides frequently determines which reproductive pathway is most appropriate.
Has the male side of the recurrent miscarriage assessment been overlooked?
A reproductive-urology assessment can review the semen results, reproductive history, relevant genetics, sperm DNA fragmentation where appropriate and potentially modifiable male fertility factors before the couple's next reproductive plan is decided.
