What Does Sex Feel Like With a Penile Implant?
Many men considering a penile implant want to know what sex will actually feel like afterwards.
Will the penis still have feeling? Can you still have an orgasm? Will your partner notice the implant? Will sex feel natural?
For most men, sex with a penile implant feels much more natural than they expected. The implant makes the penis firm enough for sex, but it does not usually change the feeling in the skin or head of the penis.
Most men can still enjoy sexual touch, penetration and orgasm.
The simple answer
A penile implant usually gives you an erection that is:
firm enough for penetration
reliable
under your control
able to last as long as you choose
not dependent on tablets or injections
The implant mainly provides firmness. It does not create sexual desire or pleasure by itself.
Your sexual feelings still come from your nerves, brain, hormones and emotional connection with your partner.
Penile implants are a well-established treatment for erectile dysfunction when tablets, injections or other treatments have not worked.[1,2]
Will my penis still have feeling?
In most men, the feeling in the penis stays much the same as it was before surgery.
The implant is placed inside the two erection chambers of the penis. It is not placed in the skin or directly in the head of the penis.
Most men can still feel:
touching and stroking
warmth and pressure
movement during penetration
stimulation of the head of the penis
There can be some temporary numbness, tingling or unusual sensations while the area is healing. This usually improves over time.
A penile implant cannot bring back feeling that was already lost because of diabetes, nerve damage, spinal cord injury or previous pelvic surgery.
Will the erection feel natural?
An inflatable penile implant usually creates a strong and firm erection.
During sex, many men say it feels similar to a natural erection. However, there are some differences.
The erection may:
feel firmer than a natural erection
stay the same level of firmness throughout sex
remain firm after orgasm
stay erect until you deflate the implant
The skin of the penis still moves normally over the implant. The penis also remains warm because blood still flows through the skin and surrounding tissues.
Your partner will usually feel a firm penis rather than the separate parts of the implant.
What happens to the head of the penis?
The implant makes the shaft of the penis firm. It does not go into the head of the penis.
The head may still become fuller during sexual excitement. However, it may remain softer than the shaft.
This is more common in men who have:
diabetes
poor blood circulation
previous prostate surgery
previous pelvic radiotherapy
severe erectile dysfunction
A softer head usually does not stop penetration, but some men notice the difference.
Can I still have an orgasm?
Most men who could have an orgasm before surgery can still have one after penile implant surgery.
An orgasm is controlled by the nerves, spinal cord and brain. The implant is not placed in these areas.
Research suggests that penile implant surgery does not usually harm a man’s ability to have an orgasm.[4,5]
However, some men already have difficulty reaching orgasm because of:
prostate cancer treatment
diabetes
nerve problems
low testosterone
antidepressants or other medications
anxiety or depression
relationship difficulties
The implant may improve confidence and reduce worry about losing an erection. This can make sex and orgasm easier for some men.
However, the implant itself does not directly create an orgasm.
What happens after orgasm?
With a natural erection, the penis usually becomes soft after orgasm.
With an implant, the erection stays firm until you deflate it.
This means you can continue sexual activity after orgasm if you and your partner wish.
You can also pause or change position without worrying that the erection will disappear.
Can I still ejaculate?
A penile implant does not usually stop ejaculation.
If you could ejaculate before surgery, you will usually still be able to ejaculate afterwards.
However, the implant cannot bring back ejaculation that was already lost.
For example, men who have had their prostate removed usually have a dry orgasm. They may still feel the pleasure of orgasm, but no semen comes out.
Ejaculation may also be affected by:
diabetes
spinal cord injury
nerve damage
prostate surgery
radiotherapy
some medications
The implant treats the erection. It does not control semen production.
Will the implant improve my sex drive?
A penile implant does not directly increase your sex drive.
Sexual desire depends on many things, including:
testosterone
general health
stress
mood
medications
confidence
your relationship
Some men notice that their interest in sex improves after surgery because they feel more confident and no longer worry about their erection failing.[6]
However, low sex drive may need separate treatment.
What will sex feel like for my partner?
Most partners report that sex feels normal or satisfying after penile implant surgery.
During penetration, your partner will usually feel a firm penis.
Your partner may notice that:
the penis feels consistently firm
the head feels softer than the shaft
the erection remains after orgasm
you need a short time to inflate the device before sex
The implant is completely inside the body.
The pump can be felt in the scrotum if someone deliberately looks for it, but it is not usually noticed during sex.
Studies have found high satisfaction rates among both patients and partners after penile implant surgery.[1,3]
Does using the implant interrupt sex?
At first, using the pump may feel unfamiliar.
You need to squeeze the pump inside the scrotum several times to create the erection. This usually takes a short time.
Many couples include this as part of foreplay.
With practice, most men become comfortable using the pump and can inflate the implant easily.
One advantage is that you do not need to:
wait for tablets to work
give yourself an injection
use a vacuum device before sex
worry about losing the erection during intercourse
Does a malleable implant feel different?
A malleable implant uses two bendable rods.
The penis stays firm all the time. You move it upwards for sex and downwards when you want to conceal it.
A malleable implant is simple to use and does not require pumping.
However, the penis does not become fully soft. This can feel less natural when you are not having sex and may be harder to hide under clothing.
Studies suggest that inflatable implants often have higher patient and partner satisfaction than malleable implants.[7] However, both types can provide a reliable erection.
The best implant depends on your health, hand strength, lifestyle and personal preferences.
Will sex hurt?
Sex should not remain painful after the operation has healed.
During the first few weeks, the penis and scrotum may feel:
swollen
bruised
tight
tender
uncomfortable when the implant is inflated
Most surgeons recommend waiting about four to six weeks before having sex. Some men need longer.
When you first return to sex, it may help to:
use plenty of lubricant
start slowly
choose a comfortable position
avoid forceful bending of the penis
give yourself time to become confident
Pain should improve over time.
Contact your surgeon if you develop increasing pain, redness, discharge, fever, difficulty using the pump or a change in the position of the implant.
Will my penis be the same size?
A penile implant is designed to make the penis firm. It is not designed to make the penis longer or wider.
The surgeon chooses the implant size based on the space available inside the penis.
Some men feel that their penis looks shorter after surgery. Often, shortening had already happened before the operation because of:
long-term erectile dysfunction
prostate surgery
Peyronie’s disease
priapism
scarring
weight gain around the lower abdomen
The implant cannot safely make the penis longer than the existing tissues allow.
Studies show that concerns about penis length are an important cause of disappointment after surgery.[4,8] For this reason, it is important to discuss expected size before the operation.
Can someone tell that I have an implant?
An inflatable implant is completely inside the body.
It is usually not noticeable through clothing.
A partner who carefully feels the penis or scrotum may notice the cylinders or pump. However, during sex, most partners mainly notice a firm erection.
Whether you tell a new partner is a personal decision.
Many men find that being open about the implant reduces anxiety and helps their partner understand why the erection stays firm after orgasm.
How satisfied are men after a penile implant?
Penile implants have some of the highest satisfaction rates of any treatment for erectile dysfunction.
The European Association of Urology reports very high satisfaction among carefully selected and properly counselled patients and their partners.[1]
A recent review of more than 12,000 patients found an overall satisfaction rate of about 83%.[3]
Most men are pleased with the reliability of the erection and the return of penetrative sex.
However, not every man is completely satisfied.
Common reasons for disappointment include:
expecting the penis to become longer
the head of the penis remaining soft
difficulty using the pump
ongoing pain
altered sensation
infection
mechanical problems
unrealistic expectations about how natural the erection will feel
