Can You Ride a Bike or Surf With a Penile Implant?
Short answer:
Most men can return to cycling and surfing after recovering from penile implant surgery.
Avoid these activities during the early healing period.

A common concern before penile implant surgery is whether the device will interfere with cycling, surfing or other physical activities.
The reassuring answer is that a penile implant is designed to remain completely inside the body. Once healing is complete, most patients can return to normal exercise and recreational activities without permanent restrictions.[1-5]
However, cycling and surfing place different types of pressure on the pelvis. Returning at the right time is important.
What is a penile implant?
A penile implant, also called a penile prosthesis, is placed inside the penis to treat erectile dysfunction that has not responded adequately to other treatments. International guidelines recognise penile implants as an established and effective treatment option.[1-4]
The two main types are:
Inflatable implant: Cylinders in the penis are connected to a pump in the scrotum and a fluid reservoir in the abdomen or pelvis.
Malleable implant: Flexible rods remain firm but can be positioned manually.
An inflatable implant becomes soft when deflated. A malleable implant remains partly rigid, which may make bicycle saddles or lying face-down on a surfboard more noticeable for some men.[1,5]
Can you ride a bike with a penile implant?
Yes, once you have healed and your surgeon has cleared you to return.
There is no strong evidence that ordinary cycling commonly damages a healed penile implant. Cycling is not identified as a frequent cause of device failure in published implant studies.[5,6,8,10]
Cycling should still be avoided during early recovery. A bicycle saddle places direct pressure on the perineum—the area between the scrotum and anus. This can increase pain, swelling and irritation around healing tissues.
Research in cyclists without implants has shown that conventional saddles can increase perineal pressure and temporarily affect penile blood flow. This does not prove that cycling damages an implant, but it supports reducing unnecessary pressure.[9]
When can you cycle after surgery?
There is no single scientifically proven timetable for returning to cycling. Postoperative instructions vary between surgeons and should be tailored to the patient and procedure.[3,4]
Many surgeons recommend avoiding bicycle saddles for approximately six weeks or longer. Additional recovery may be needed after:
Revision or complex implant surgery
Significant swelling or bruising
Ongoing penile, scrotal or perineal pain
Surgery involving Peyronie’s disease or severe fibrosis
Delayed wound healing
Previous pelvic surgery or radiation
Your surgeon should confirm that the wounds have healed, the implant is positioned correctly and the device is functioning normally.
Returning to cycling safely
Begin with a short session on a stationary bicycle after receiving surgical clearance. Use low resistance and stop if you experience discomfort.
Gradually increase the duration before increasing intensity. Return to smooth outdoor surfaces before progressing to mountain biking, rough trails or jumps.
How can cyclists reduce pressure?
Keep the implant deflated
An inflatable implant should generally remain deflated while cycling. This makes the penis easier to position and reduces rigidity against the saddle.
Review your bike fit
Incorrect saddle height, handlebar position or riding posture can concentrate pressure on the front of the saddle. A professional bike fit may help transfer weight towards the sitting bones.
Choose a comfortable saddle
A wider or pressure-relieving saddle may be helpful. A systematic review found that no-nose saddles and recumbent bicycles reduced perineal pressure more effectively than standard saddles. However, no single saddle design works for every rider.[9]
Stand and change position
Standing briefly on the pedals removes pressure from the perineum. Change position regularly and take breaks during longer rides.
Do not ignore numbness or pain
Penile or perineal numbness means pressure should be relieved. Persistent pain, numbness or swelling should be assessed rather than accepted as a normal part of cycling.
Can you surf with a penile implant?
Most men can return to surfing after complete recovery.
No high-quality studies have directly examined surfing after penile implant surgery. However, normal paddling, sitting on a board and moving from lying to standing would not usually be expected to damage a correctly positioned, healed implant.
The main concerns during early recovery include:
Pressure from lying face-down on the board
Repeated pelvic and abdominal movement while paddling
Sudden twisting during take-off
Impact from the board, fins or shallow water
Sand or seawater reaching an incompletely healed wound
Implant infection can require further surgery. Do not enter a pool or the ocean while an incision remains open, moist, inflamed or incompletely healed.[1,4,8,10]
When can you surf after surgery?
Wait until:
The wounds have fully healed
Swelling and bruising have substantially settled
There is no redness, discharge or increasing pain
Lying face-down and paddling movements are comfortable
The implant can be inflated and deflated normally
Your surgeon has approved your return to water and sport
For many patients, this will be at least six weeks after surgery, although some need longer.
Start with a short session in small, predictable conditions rather than immediately returning to heavy surf or long-distance paddling.
Should the implant be inflated while surfing?
An inflatable implant should normally remain deflated while surfing. A deflated penis is generally easier to position beneath a wetsuit and against the board.
With a malleable implant, the penis can usually be directed downward or to the side. Persistent pressure, rubbing or skin irritation should be discussed with the implant surgeon.
Can a wetsuit affect a penile implant?
A properly fitted wetsuit should not damage a healed implant. It should feel supportive rather than painfully tight.
Check that:
The penis is positioned comfortably
The scrotal pump is not pressed against a seam
There is no focal rubbing over an incision
You can squat and perform a surfing take-off without pain
Pain, numbness, skin blanching or significant pressure suggests that the wetsuit fit or implant positioning should be reviewed.
Can an accident damage the implant?
Modern implants are durable, but significant trauma to the penis, scrotum, pelvis or lower abdomen could injure the surrounding tissues or an implant component.
Seek medical assessment after an accident if you develop:
New or increasing penile or scrotal pain
Rapid swelling or bruising
A change in pump position
A new bend, bulge or deformity
Difficulty inflating or deflating the implant
Loss of implant rigidity
Skin thinning or exposure of a device component
Will exercise shorten the life of the implant?
There is no evidence that normal cycling or surfing after complete healing significantly shortens implant lifespan.
Long-term studies show that implant components can eventually wear, leak or malfunction. One contemporary series reported estimated device survival of approximately 87% at five years and 78% at ten years.[6] Ordinary recreational exercise was not identified as a major cause of failure.

Can riding a bicycle break a penile implant?
Ordinary cycling after recovery is not recognised as a common cause of implant failure. New pain or a sudden change in implant function should be assessed.
Is a stationary bicycle safer?
A stationary bicycle allows better control of resistance and duration, making it useful for a gradual return. It still places pressure on the perineum and should be avoided during early recovery.
Can I mountain bike with an implant?
Usually, once fully healed. Become comfortable with ordinary cycling before returning to rough terrain, jumps or activities with a higher risk of falling.
Can salt water infect an implant?
Salt water cannot reach a completely healed implant. Entering the ocean before the incision has fully healed may expose the wound to bacteria.
Does a penile implant limit an active lifestyle?
For most appropriately selected patients, the long-term goal is a return to normal life rather than permanent restriction.
Studies report high levels of functional, personal and relationship satisfaction after penile prosthesis surgery.[2,3,5,7] Men who cycle, surf or perform demanding physical work should discuss these activities before surgery so the procedure and recovery plan can be tailored to their lifestyle.
