What to bring

  • Medical insurance information

  • Referral letter

  • Copies of blood results, MRIs, CT scans etc and any other relevant information

  • List of medications (if any)

  • If attending for penile curvature - please bring photographs of the erect penis looking down and from the side to allow assessment and documentation of curvature

On arrival

Prior to your consultation with Dr Crozier, you will be requested to complete a patient registration form. Please arrive 5 minutes early to allow for this registration process.

Cancelling appointments
